ABSTRACT

Objective: To investigate the epidemiological characteristics of pneumoconiosis deaths in Putuo District of Shanghai, and to provide evidence for prevention and management of pneumoconiosis in the future. Methods: In June 2021, the data of 263 patients with pneumoconiosis who died from January 1961 to December 2020 in Putuo District of Shanghai were retrospectively analyzed. Trend χ(2) test, analysis of variance and Spearman rank correlation methods were used to analyze the basic information, age of exposure to dust, promotion period, course of disease and direct cause of death of patients with pneumoconiosis. Results: Among the 263 cases of pneumoconiosis patients who died in Putuo District of Shanghai from 1961 to 2020, 260 cases (98.86%) were male. The main types of pneumoconiosis were foundry worker pneumoconiosis (53.23%, 140/263) and silicosis (43.73%, 115/263). The main types of pneumoconiosis were sand cleaner (36.50%, 96/263). The age of onset was (53.42±10.13) years old, the age of death was (76.14±9.12) years old, and the age of exposure to dust was (20.91±8.99) years. The duration of dust exposure was negatively correlated with the duration of disease (r(s)=-0.24, P<0.001). With the increase of pneumoconiosis stage, the mortality of silicosis and foundry worker pneumoconiosis also showed an increasing trend (χ(2)(trend)=4.22, 3.87, P=0.040, 0.049). 31.94% (84/263) of pneumoconiosis patients died directly from pneumoconiosis, ranking first among the direct causes of death. Conclusion: The death cases of pneumoconiosis in Putuo District of Shanghai are mainly foundry worker pneumoconiosis and silicosis, and pneumoconiosis is the main cause of death.

ABSTRACT

OBJECTIVE@#Exposure to high intensity, low frequency noise (HI-LFN) causes vibroacoustic disease (VAD), with memory deficit as a primary non-auditory symptomatic effect of VAD. However, the underlying mechanism of the memory deficit is unknown. This study aimed to characterize potential mechanisms involving morphological changes of neurons and nerve fibers in the hippocampus, after exposure to HI-LFN.@*METHODS@#Adult wild-type and transient receptor potential vanilloid subtype 4 knockout (TRPV4-/-) mice were used for construction of the HI-LFN injury model. The new object recognition task and the Morris water maze test were used to measure the memory of these animals. Hemoxylin and eosin and immunofluorescence staining were used to examine morphological changes of the hippocampus after exposure to HI-LFN.@*RESULTS@#The expression of TRPV4 was significantly upregulated in the hippocampus after HI-LFN exposure. Furthermore, memory deficits correlated with lower densities of neurons and neurofilament-positive nerve fibers in the cornu ammonis 1 (CA1) and dentate gyrus (DG) hippocampal areas in wild-type mice. However, TRPV4-/- mice showed better performance in memory tests and more integrated neurofilament-positive nerve fibers in the CA1 and DG areas after HI-LFN exposure.@*CONCLUSION@#TRPV4 up-regulation induced neurofilament positive nerve fiber injury in the hippocampus, which was a possible mechanism for memory impairment and cognitive decline resulting from HI-LFN exposure. Together, these results identified a promising therapeutic target for treating cognitive dysfunction in VAD patients.

ABSTRACT

OBJECTIVES@#To investigate the prevalence of polypharmacy and potentially inappropriate medication (PIM) in elderly patients with heart failure (HF) and their impact on readmission and mortality.@*METHODS@#We conducted a study of 274 participants aged 60 years or older with HF. The prevalence of polypharmacy (defined as the use of five or more medications) was calculated, and the 2019 American Geriatrics Society Beers criteria were applied to access PIMs. Medications and PIMs were characterized at admission and discharge, and changes in prescriptions during hospitalization were compared. The impact of polypharmacy and PIM on readmission and mortality were investigated.@*RESULTS@#The median age of this study population was 68 years old. The median number of prescribed drugs was 7 at admission and 10 at discharge. At discharge, 99.27% of all patients were taking five or more drugs. The incidence of composite endpoint and cardiovascular readmission increased with the number of polypharmacy within 6 months. The use of guideline-directed medical therapy reduced the incidence of composite endpoint events and cardiovascular readmission, while the use of non-cardiovascular medications increased the composite endpoint events. The frequency of PIMs was 93.79% at discharge. The incidence of composite endpoint events increased with the number of PIMs. "PIMs in older adults with caution" increased cardiovascular readmission and "PIMs based on kidney function" increased cardiovascular mortality. Several comorbidities were associated with cardiovascular mortality or non-cardiovascular readmission.@*CONCLUSIONS@#Polypharmacy and PIM were highly prevalent in elderly patients with HF, and their use was associated with an increased risk of composite endpoint events, readmission and mortality. Non-cardiovascular medications, "PIMs in older adults with caution", "PIMs based on kidney function" and several comorbidities were important factors associated with hospital readmission and mortality. Our findings highlight the importance of medication optimization in the management of HF in elderly patients.

ABSTRACT

ObjectiveTo explore the active ingredients, therapeutic targets, and relative signaling pathways of Tripterygium wilfordii in the treatment of triple negative breast cancer (TNBC) based on network pharmacology, and to verify the mechanism through in vitro cell model. MethodThe active ingredients of T. wilfordii were screened from the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP). The targets of TNBC were obtained from DisGeNET and GeneCards. Venny was used to identify the potential therapeutic targets of T. wilfordii against TNBC. Protein-protein interaction (PPI) network was constructed with String database. Gene ontology (GO) annotation and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment were carried out with DAVID to predict the mechanisms of potential targets. The molecular docking between triptolide and key targets were performed with AutoDock Vina. The effect of triptolide (0, 5, 10, 20, 30, 40, 50, 60, 80 nmol·L-1) on the proliferation of MDA-MB-231 cells was determined through methyl thiazolyl tetrazolium (MTT) assay. The effect of triptolide (0, 12.5, 25, 50 nmol·L-1) on the apoptosis of MDA-MB-231 cells was detected with Hoechst 33342 staining. Western blot was performed to detect the effect of triptolide (0, 25, 50 nmol·L-1) on the expression levels of key targets. ResultT. wilfordii had 23 active ingredients related to 55 potential targets of TNBC. GO and KEGG enrichment revealed that the potential targets were associated with 103 biological processes, 15 cellular components, and 35 molecular functions, and were involved in 140 signaling pathways including atherosclerosis and apoptosis. The results of molecular docking demonstrated that triptolide could bind with the targets including threonine kinase 1 (Akt1), vascular endothelial growth factor A (VEGFA), cellular tumor antigen p53 (p53), transcription factor AP-1 (JUN), signal transducer and activator of transcription 3 (STAT3), tumor necrosis factor (TNF), mitogen-activated protein kinase 8 (MAPK8), prostaglandin G/H synthase 2 (PTGS2), and Caspase-3. According to the results of MTT assay, triptolide (20, 30, 40, 50, 60, 80 nmol·L-1) inhibited the proliferation of MDA-MB-231 cells compared with blank control (P<0.05, P<0.01). Hoechst 33342 staining showed that triptolide (12, 25, 50 nmol·L-1) induced the apoptosis of MDA-MB-231 cells compared with black control (P<0.05, P<0.01). Western blot showcased that 50 nmol·L-1 triptolide down-regulated the relative expression levels of p-Akt, TNF-α, and VEGFA, while 25 and 50 nmol·L-1 triptolide up-regulated the relative expression level of p53 in a dose-dependent manner compared with the blank control (P<0.05, P<0.01). ConclusionT. wilfordii has multiple ingredients, targets, and pathways in the treatment of TNBC. It may regulate p53, VEGFA, TNF-α and other key targets to induce cell apoptosis and suppress angiogenesis and inflammatory response, which provides a scientific basis for the further investigation and clinical application of T. wilfordii.

ABSTRACT

@#AIM:To explore the correlation between maternal iron deficiency anemia and retinopathy of prematurity(ROP)in premature infants or low birth weight infants so that to provide possible scientific basis for the prevention and control of ROP.<p>METHODS: This study was a case-control study. The clinical data of 317 premature or low birth weight infants who were diagnosed with ROP and their mothers in our hospital during January 2019 to July 2021 due to ROP screening for the first time(about 30d after birth)were analyzed. The relationship between maternal iron deficiency anemia and ROP and its stages were observed. And the relationship between Hb, blood value characteristics, mean gestational age, gestational weeks, infant birth weight of maternal iron deficiency anemia and ROP stage.<p>RESULTS: There were 235 mothers(74.1%)with iron deficiency anemia, 82 mothers(25.9%)without iron deficiency. Among them, there were 194 mothers(82.6%)with mild anemia according to anemia classification, 119 cases with stage Ⅰ ROP and 75 cases with stage Ⅱ ROP. There were 39 mothers(16.6%)with moderate anemia, 8 cases with stage Ⅱ ROP, 22 cases with stage Ⅲ ROP and 9 cases with stage Ⅳ ROP. There were 2 mothers(0.9%)of severe anemia, all of whom had stage Ⅳ ROP. No children with stage Ⅴ or threshold ROP and mothers with very severe anemia were found in this study. Compared with mothers with iron deficiency anemia, premature infants or low birth weight infants with normal iron levels were more likely to have stage Ⅰ ROP, but stage Ⅱ ROP was more pronounced in mothers with iron deficiency anemia, and the differences were statistically significant(all <i>P</i><0.05). Stage Ⅲ and stage Ⅳ ROP was not observed in the mothers with normal serum iron, but was 9.4% and 4.7% in the mothers with iron deficiency anemia, respectively. The differences were statistically significant(<i>P</i><0.05). Stage Ⅴ and threshold lesions ROP was not observed in preterm or low birth weight infants in mothers with normal serum iron values or iron deficiency anemia. Compared with mothers with normal iron levels, mothers with iron deficiency anemia had significantly lower hemoglobin, hematocrit, mean erythrocyte volume, serum iron and ferritin levels. At the same time, the higher mean gestational age, mean shorter gestational week and mean lower birth weight in the mothers with iron deficiency anemia, and the differences were statistically significant(all <i>P</i><0.05).<p>CONCLUSION:Pregnant iron-deficiency anemia is associated with the occurrence and development of ROP in premature or low birth weight infants. The more severe maternal anemia, the higher maybe stage of ROP. Therefore, monitoring and supplementation of iron during pregnancy can effectively prevent and reduce the risk of ROP.

ABSTRACT

BACKGROUND@#Despite almost two decades of well-funded and comprehensive response efforts by the Chinese Government, human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) remains a major problem in China. Yet, few studies have recently examined long-term trends in HIV/AIDS prevalence, incidence, and mortality at the national level. This study aimed to determine the prevalence, incidence, and mortality trends for HIV/AIDS over the past 28 years in China.@*METHODS@#We conducted a descriptive, epidemiological, secondary analysis of the Global Burden of Diseases, Injuries, and Risk Factors Study 2017 data. To evaluate trends in prevalence, incidence, and mortality over the study period from 1990 to 2017, we calculated values for annual percentage change (APC) and corresponding 95% confidence intervals (CIs) using joinpoint regression analysis.@*RESULTS@#A significant increase in HIV/AIDS prevalence was observed for 1990 to 2009 (APC: 10.7; 95% CI: 10.4, 11.0; P < 0.001), and then remained stable for 2009 to 2017 (APC: 0.7; 95% CI: -0.3, 1.7; P = 0.1). A significant increase in HIV incidence was also observed for 1990 to 2005 (APC: 13.0; 95% CI: 12.6, 13.4; P < 0.001), and then a significant decrease was detected for 2005 to 2017 (APC: -6.5; 95% CI: -7.0, -6.1; P < 0.001). A significant increase in AIDS-related mortality rate was detected for 1990 to 2004 (APC: 10.3; 95% CI: 9.3, 11.3; P < 0.001), followed by a period of stability for 2004 to 2013 (APC: 1.3; 95% CI: -0.7, 3.3; P = 0.2), and then another significant increase for 2013 to 2017 (APC: 15.3; 95% CI: 8.7, 22.2; P < 0.001).@*CONCLUSIONS@#Although prevalence has stabilized and incidence has declined, AIDS-related mortality has risen sharply in recent years. These findings suggest more must be done to bring people into treatment earlier, retain them in treatment more effectively, actively seek to reenter them in treatment if they dropout, and improve the quality of treatment and care regimens.

ABSTRACT

The articles regarding needle-embedding treatment for hemifacial spasm published before September 30, 2019 were searched from SinoMed, Wanfang, CNKI, VIP and PubMed database, and were analyzed and summarized from treatment methods, acupoint selection, stage differentiation and action mechanism. As a result, 45 Chinese articles were obtained. The needle-embedding treatment was divided into intradermal needling and acupoint thread-embedding; the top five acupoints were Sibai (ST 2), Taiyang (EX-HN 5), Dicang (ST 4), Jiache (ST 6) and spasm trigger points. The basic research of needle-embedding treatment for hemifacial spasm is weak, and the literature regarding stage differentiation is insufficient, which are in need of further study.

ABSTRACT

To find antibacterial candidate compounds, eighteen novel sulfonamide derivatives containing a fused-ring were designed and synthesized on the basis of previous studies, with structures confirmed by 1H NMR, 13C NMR and MS. Antibacterial activities of the products were evaluated by the agar dilution method. The results show that these derivatives have different degrees of inhibitory activity on the tested bacteria, with the compounds IIi and IIr the most potent. The MIC of IIi for S. aureus, E. coli and MRSA was 8, 32 and 16 μg·mL-1, respectively, and the MIC of the IIr was 8, 64 and 32 μg·mL-1, respectively. The anti-MRSA activities of the two compounds is significant and is worthy of further structural optimization and study.

ABSTRACT

Fatigue refers to the manifestation of disorders in the process of carrying out or maintaining random activities, which can be regarded as an independent disease or as a symptom in a variety of chronic diseases. The high incidence of fatigue has seriously affected people's physical and mental health, and the prevention and treatment of fatigue has become an important problem to be solved urgently. The pathogenesis of fatigue mainly includes energy consumpation, accumulation of metabolites, abnormal secretion of neurotransmitters, decline of mitochondrial function, dysfunction of hypothalamus pituitary adrenal axis, etc. At present, there is no unified understanding about the pathogenesis of fatigue at home and abroad. The gene research of fatigue is the current research frontier. Gene expression profiling provides a new method for the study of the mechanism of fatigue. The combination of gene chip technology and traditional Chinese medicine(TCM) theory is expected to bring a breakthrough in the study of the pathogenesis of fatigue. In the study of fatigue gene chip, messenger RNA(mRNA) and microRNA(miRNA) are the common research objects, but few explorations are focused on the gene expression rule of fatigue by a specific signaling pathway and the effective regulation targets of TCM for treating fatigue. In recent years, the dysfunction of reward and inhibition mechanism in the central nervous system has become a research hotspot. In particular, gamma amino butyric acid (GABA) and dopamine (DA) have attracted much attention as the main substances of inhibition and reward mechanism, respectively. GABA and DA are used as inhibition and reward mechanisms to maintain the balance, and the body will not feel fatigue. Once the balance is broken, the fatigue will be formed. At the same time, DA and GABA receptors can also regulate cyclic adenosine monophosphate signaling pathway(cAMP) to affect fatigue. The research on key genes in GABA/DA balance mechanism and related cAMP signaling pathway by gene chip technology is expected to reveal the pathogenesis of fatigue in depth. The gene chip method is used to detect the changes of key genes in GABA/DA pathway and the related cAMP signaling pathway in the fatigue population and the normal population, so as to further explore the pathogenesis of fatigue. In this paper, the key genes in GABA/DA balance mechanism and cAMP signaling pathway related to fatigue were summarized by using the review method, so as to provide the basis for further study on the pathogenesis of fatigue and effective prevention and treatment from the perspective of genetics.

ABSTRACT

Objective To investigate the distribution, drug resistance and molecular biological characteristics of carbapenem-resistant Klebsiella pneumoniae (CRKP) in our hospital, so as to provide reference for rational use of antibiotics and prevention and control of nosocomial CRKP infection. Methods Non-repetitive CRKP strains were collected from Jan. to Dec. 2019 in our hospital. VITEK 2 Compact automatic microbial analyzer and Kirby-Bauer test were used for bacterial identification and antimicrobial susceptibility analysis. WHONET 5.6 software was used to analyze CRKP detection rate, sample source and clinical department distribution. Hypermucoviscosity phenotype strains were screened by string test. Carbapenemase resistance genes, capsular serotype and virulence genes were detected by polymerase chain reaction (PCR). Results A total of 532 Klebsiella pneumoniae strains were detected, including 140 (26.3%) CRKP strains. The CRKP strains were mainly isolated from sputum and bronchoalveolar lavage fluid (66 strains, 47.1%), followed by urine (21 strains, 15.0%). The clinical departments of the isolates were mainly cardiovascular surgery intensive care unit (ICU) (47 strains, 33.6%), burn ICU (18 strains, 12.9%) and emergency department (18 strains, 12.9%). The antimicrobial susceptibility test showed that the CRKP strains were susceptible only to tigecycline, with resistance rates being over 50% to other common antibiotics. The resistance rates to the first to fourth generation cephalosporin antibiotics were above 85%, and the resistance rates to carbapenems were up to 100.0%. We also found that out of the 121 CRKP strains, 101 (83.5%) carried Klebsiella pneumoniae carbapenemase 2 (KPC-2) gene, seven (5.8%) with oxacillinase-48 (OXA-48) gene, and two (1.7%) with New Delhi metallo-β-lactmase 1 (NDM-1) gene; while one carried both KPC-2 and NDM-1 genes, and one carried both KPC-2 and OXA-48 genes; and nine carried no target drug-resistance genes. Fifteen (12.4%, 15/121) CRKP strains were positive for string test, with 13 being K64 capsular type and two being K47 capsular type; and 14 strains carried at least one virulence gene. Conclusion The clinical isolation rate of CRKP is high in our hospital, and the CRKP strains (mainly K64 capsular high virulence) are resistant to multiple antibiotics, suggesting that we should further strengthen the monitoring of drug resistance and rational use of antibiotics, so as to prevent the spread and prevalence of drug-resistant and highly virulent strains.

ABSTRACT

@#AIM: To research the correlation between the concentrations of interleukin-6(IL-6)and monocyte chemokine-1(MCP-1)in aqueous humor and the intravitreal ranibizumab injection was injected into the glass body cavity.<p>METHODS: Forty patients(40 eyes)diagnosed with RVO macular edema were selected as the treatment group, twenty patients(20 eyes)underwent cataract surgery were selected as the control group, anterior aqueous humor was collected before surgery in the treatment group and control group. Using the cytometric bead array methods detection the concentration of MCP-1 and IL-6. Comparison and analysis the concentration of MCP-1 and IL-6 before the operation treatment group and control group, comparison and analysis the concentration of MCP-1 and IL-6, BCVA, CMT before and after the operation in the treatment group. <p>RESULTS: The concentration of MCP-1(49.84±16.16、547.75±108.45pg/mL)and IL-6 in the treatment group was higher than the control group(10.71±7.26、235.65±34.45pg/mL,all <i>P</i><0.001)before the operation. There was a positive correlation between the concentration of MCP-1, IL-6 and CMT before the operation in the treatment group(<i>r</i>=0.646, 0.912, all <i>P</i><0.001). The concentration of IL-6 was significantly correlated with MCP-1(<i>r</i>=0.902, <i>P</i><0.001). The treatment group underwent the intravitreal ranibizumab injection was injected into the glass body cavity, the concentration of IL-6, MCP-1(20.08±11.56、228.35±70.69pg/mL)was lower than before. BCVA was improved significantly compared with before operation, CMT decreased after surgery compared to before surgery(<i>P</i><0.001).<p>CONCLUSION: There was a positive correlation between the concentration of MCP-1, IL-6 and CMT, with IVR treatment of secondary macular edema to RVO, to reduce the concentration of MCP-1 and IL-6, to reduce CMT, and reduce macular edema, improved patient's vision level.

ABSTRACT

Objective:To study the effect of chair inclination angles forward on sit-to-stand time and muscle activation of lower extremities in stroke patients with hemiplegia. Methods:From January to August, 2019, 15 stroke patients with hemiplegia finished five times sit-to-stand at seat slope 0°, 10° and 20° forward. The time, and surface electromyogram (sEMG) signals of rectus femoris, hamstrings, anterior tibialis and peroneus muscle were recorded. Results:The total EMG peak, root mean square and integrated electromyography (iEMG) of all the muscles decreased at seat slope 10° and 20° forward compared with those at 0° (F > 4.530, P < 0.05). The time decreased at seat slope 20° forward compared with that at 0° (P < 0.05). Conclusion:Seat inclination forward at some angles may improve the sit-to-stand performance in stroke patients with hemiplegia.

ABSTRACT

Objective To investigate the prevalence and risk factors of Blastocystis hominis infections among patients with HIV/AIDS in Fuyang City, Anhui Province. Methods A cross-sectional study was conducted in Fuyang City, Anhui Province in 2016. The demographic and socioeconomic status, and the lifestyle and production style were collected using a questionnaire survey. B. hominis DNA was detected in subjects’stool samples using a PCR assay, and the CD4+ T lymphocyte count and HIV viral load were measured in the subjects’ blood samples. The risk factors of B. hominis infections among patients with HIV/AIDS were identified using univariate and multivariate logistic regression analyses. Results A total of 398 HIV/AIDS patients were enrolled in this study, with a mean age of 49.3 years, a mean body weight of 55.9 kg and a mean height of 164.4 cm. The prevalence of B. hominis infection was 6.78% in the study subjects, and no gender- (χ2 = 1.589, P = 0.207), education level- (χ2 =0.508, P = 0.776), marital status- (χ2 = 0.419, P = 0.811) or occupation-specific prevalence (χ2 = 2.744, P = 0.615) was detected. Among the patients with HIV/AIDS, there were no significant differences in the age (t = 0.370, P = 0.712), height (t = 1.587, P =0.113), body weight (t = 0.516, P = 0.606), CD4+ T lymphocyte count (t = 1.187, P = 0.230) or HIV viral load (t = 0.193, P =0.496) between B. hominis-infected and uninfected individuals. Dinking non-tap water [OR = 6.554, 95% CI: (1.876 to 22.903)] and keeping dogs [OR = 5.895, 95% CI: (2.017 to 17.225)] were identified as risk factors for B. hominis infection in patients with HIV/AIDS. Conclusion The prevalence of B. hominis infection is high in HIV/AIDS patients, and drinking non-tap water and keeping dogs are risk factors for B. hominis infection among HIV/AIDS patients.
